Advance Care Planning
Advance Care Planning is a conversation between patients, their families, and care providers about the patient’s goals, values and beliefs to inform current and future care. A person’s life experiences can determine their decisions, and preferences may change over time in response to their experiences.
As part of this conversation, a person may wish to establish their wishes in writing by completing an advance directive.
Advance directive documents can vary from state to state. You can find out more about your state’s requirements here.
