Services

DCI provides the highest standard of care from late-stage chronic kidney disease (Stages 4 and 5) through your choice of treatment and when needed, end-of-life care. We tailor our services to meet you where you are in your kidney health journey and take active measures to help you understand your medical condition, treatment choices, financial options, and the resources available to you.

Patient-Centered Services

DCI is dedicated to patient care and ensuring that you have the resources you need on your kidney health journey.

As you explore your treatment options, think about what would work best for your life and your family. The decision is yours, even if you are already on another type of dialysis. Your treatment choice is never final; conversations with your nephrologist may be helpful in finding your best option.

We are here to empower you with the knowledge you need to make your decision. No matter which option you choose, our care team is here to support you and answer your questions.

Partner-Focused Services

We recognize that many providers must develop strategies to maintain and improve the quality and scope of their services while facing increasing fiscal and competitive pressures. We partner with hospitals, health systems, and other providers to support a sustainable kidney care program in their community.

REACH Kidney Care, a service line of DCI, provides one-on-one kidney health management services. The REACH care team will work with you to understand your life goals and your doctor’s prescribed treatment plan. REACH aims to support better kidney health by increasing kidney transplants, delaying dialysis as long as possible (starting no earlier than is medically necessary), increasing home-based dialysis, increasing permanent dialysis accesses, and decreasing hospitalization at the start of dialysis.
