Kidney Health Management Services

If you have been diagnosed with chronic kidney disease but aren’t sure what that means, or you have been told that you will eventually need dialysis and want to know how you can delay the need for it, REACH Kidney Care, our kidney health management service line, can help. REACH helps patients with advanced kidney disease navigate this life changing transition. We partner with you and your doctor to support your kidney health before dialysis is needed, with the primary goal of delaying or even avoiding the need for dialysis through an early kidney transplant.

DCI provides kidney health management services through its REACH Kidney Care service line. REACH currently operates in select states where DCI provides dialysis services. REACH aims to support better kidney care during the advanced stages of kidney disease. REACH goals for better care include increasing kidney transplants, delaying the start of dialysis as long as possible (starting no earlier than needed), increasing home-based dialysis, increasing permanent dialysis accesses, and avoiding hospitalization at the start of dialysis.

REACH services include patient-centered education and care coordination; dietary, lifestyle, and psychosocial guidance; medication support; patient-centered life goals and advance care planning; and optimal transition support.
